Beakerz Participation Agreement

 

About the Program

 

Beakerz Science LLC ("Beakerz") provides hands-on science, technology, engineering, and math enrichment classes for children. Classes are led by Beakerz instructors, who may be employees or independent contractors engaged by Beakerz. Beakerz provides the curriculum, instruction, and materials. Beakerz does not operate the Facility and is not responsible for the Facility's premises, staffing, policies, or supervision outside of Beakerz class time.

 

Nature of the Activities

 

Beakerz classes are hands-on and experimental by design. Children may handle, mix, pour, build, and observe age-appropriate materials, which can include (among others) water, food-grade ingredients, baking soda and vinegar, food coloring, soap, glue, craft supplies, magnets, small building components, balloons, and simple tools. Activities may involve movement, mess, spills, staining of clothing, and incidental contact between children.

 

I understand that hands-on science activities carry inherent risks that cannot be entirely eliminated, including but not limited to minor cuts, scrapes, bumps, slips and falls, skin or eye irritation, allergic reaction, choking on small parts, and soiled or stained clothing.

 

Allergies, Sensitivities, and Health

 

I will disclose any allergies, sensitivities, medical conditions, or other needs relevant to my child's participation:

 

I understand that Beakerz materials are not guaranteed to be free of common allergens, that classes take place in a shared environment Beakerz does not control, and that it is my responsibility to notify the Facility and Beakerz in writing of any allergy or condition, and to update that information if it changes.

 

I authorize Beakerz instructors to seek emergency medical care for my child if needed, and I accept financial responsibility for any such care. Beakerz instructors do not administer medication, including epinephrine auto-injectors, and will direct all medical needs to Facility staff.

 

 

Supervision

 

Beakerz classes are conducted on Facility premises during Facility hours. Facility staff remain responsible for the general care, custody, supervision, and safety of enrolled children at all times, including during Beakerz class time. Beakerz instructors are responsible for delivering instruction and for the safe conduct of the science activities.

 

 

No Guarantee of Outcomes

 

Beakerz classes are enrichment programming. Beakerz makes no representation or guarantee regarding academic outcomes, test performance, developmental milestones, or educational results. Beakerz does not provide medical, therapeutic, diagnostic, or special-education services, and nothing in the program should be treated as a substitute for professional evaluation or care.

 

Assumption of Risk and Release

 

Knowing the nature of the activities described above, and on behalf of myself, my child, and our heirs and representatives, I voluntarily assume the ordinary risks of participation. To the fullest extent permitted by applicable law, I release and agree not to sue Beakerz Science LLC, its owners, officers, employees, and independent contractors for claims arising from the ordinary risks of participation in the program.

 

This release does not apply to, and I am not waiving, any claim arising from gross negligence, recklessness, or intentional or willful misconduct. Nothing in this agreement limits any right that cannot be waived under applicable law.

 

If any provision of this agreement is found unenforceable, the remaining provisions remain in full effect.

 

Home Extension Activities

 

Any take-home materials or suggested at-home activities are intended to be completed under direct adult supervision. Beakerz is not responsible for injury or damage resulting from activities conducted outside of a Beakerz class.

 

Governing Law

 

This agreement is governed by the laws of the state in which the class is held, without regard to conflict-of-laws principles.

 

I have read this agreement, understand it, and accept it voluntarily.
