Liability Waiver
By completing this purchase, you acknowledge and agree to the following:
I, the parent or legal guardian of the enrolled student, hereby grant permission for my child to participate in Beakerz in-school science classes. I understand that science activities may involve materials and experiments, and I agree to hold Beakerz, its instructors, and affiliated schools harmless from any injury, loss, or damage that may occur during class activities. I confirm that the student information provided is accurate and that I am authorized to enroll this student. I understand that the $35 registration fee is non-refundable.
By proceeding with payment, you confirm that you have read, understood, and agree to this liability waiver.

After completing your purchase, you will be contacted to provide your student's details. Alternatively, please include the following in the order notes at checkout:
- Student's full name
- Student's grade
- Student's age
- School name
- Class time/session
- Any allergies or medical conditions we should be aware of
- Emergency contact name and phone number
